This attached membership agreement form is intended in its entirety, to inform you, the patient,
of the regulations and commitments for an IV membership to Healing Hydration, LLC.
· Either (1) 6 month or (1) 12 month commitment is required for the membership. If any
membership cancelation is desired before the 6 or 12 month time limit expires, a $100
cancelation fee will be charged to the card on file.
· Each membership must be linked to a credit card on file, as this will be the credit card that is charged each month throughout the duration of the membership.
· If credit card processing does not go through on the monthly charge, an additional
5% fee will be added on to total invoice, and any IV treatment will not be
able to be redeemed until payment is received.
· If the monthly charge does not come out on the 1st of each month, the charge for the monthly IV has to go through prior to the monthly IV being scheduled and carried out.
· The credit card chosen to be on file will be charged once monthly.
· The patient is responsible for scheduling their monthly IV service.
· If an IV is not used within the month, it will be lost. If there is an unforeseen
circumstance that takes place prohibiting that month’s IV to be used, please speak with
the owner, Aubrey, about the situation for further arrangements to be made.
· If more than one IV is desired/needed within one month, the membership price will be honored for all additional IVs received.
· Any additional IV’s received within the same month will come with the free add on's
· If you would like to gift an IV to a friend or family member for any one of the months
during the membership, please let Aubrey know who will be receiving the IV and which
month is preferred.
· By signing this form, you have agreed to the listed regulations and commitments above.
